If Chapter 26 is Max for Live, this should be a pretty easy mapping (although you do it inside the device, rather than using Live's MIDI mapping stuff). I'm finally getting a DX200 on Thursday and can pass you a quick illustration of the technique then (assuming that you still need it -- this is a kind of old thread).

Please let us know how it goes, and describe what you have to do to make it work. I don't know whether the OP still needs this, but it would be good to have this explained for anyone that might be looking for it. :)

OK, my DX200 is here, pretty sweet machine. I'll need to resolder or replace the encoder, but that sounds like par for the course with this box. Sounds amazing. I have a _lot_ of Yamaha FM synths, and this is quite a departure.

So here's what you need to do. First go to Shift+MIDI Setup and ensure that CC IO is set to All. Otherwise the parameters in question are ignored. You can see that info on p124-125 of the manual, where the MIDI implementation is described (the parameters are not in grey and are therefore ignored in 'Norm' mode).

Then just grab the device attached to this post and load it in Live. You'll want to drop it on a MIDI track, arm the track for record so that MIDI comes in (if you want to play notes) and set the "MIDI to" to your DX200. Then twiddle the knobs and enjoy playing with those parameters.

Here's my done-in-2-seconds interface design (it's all spread out like that to avoid using advanced features like Presentation Mode to create a compact UI view):
